2023/24 Required Sports Medical Forms -

Athletic Release Form

Medical History Form

Original Copies of the Medical Forms are required for all students participating in St. Anthony Cathedral Basilica School Athletics including Sports Teams, Camps/Clinics & Tournaments. 

Information:

• First form (1 page) is to be filled out completely by a parent/guardian. 

• Second form

  • 1st page is to be filled out completely and signed by a physician.

  • 2nd page is to be filled out completely by a parent/guardian for the doctor

    • New physicals must be filled out for the 23/24 school year prior to the start of participation.

    • Physicals need to be up to date within one year of the full season of activity. 

      • For any physicals that go beyond one year from the date of examination during the playing season, we ask that an appointment is scheduled in advance and within one week of the expiring date to avoid any missed playing time. 

Turning In Forms: 

• Only the Catholic Schools Medical Forms will be accepted for both the parent forms and physicals.

• Original copies of both forms are to be turned in to Simon Guillory prior to the below dates for the 2023/24 school year: 

  • Fall Sports - July 15th, 2023

  • Winter and Spring Sports - October 15th, 2023
